500 mg PO TID x 48h (in combination with metronidazole 500 mg PO TID)
12.5 to 25 mg/kg/day given as a single daily dose or in divided doses PO Q12H
Reconstitution instructions for 250 mg/5 mL powder for oral suspension:
1. Shake bottle to loosen powder
2. Add 60mL of sterile water in 2 portions to make a final volume of 100mL. Shake well in between additions of water
3. Shake well
50 mg/mL = 14 days
0 - 10 eGFR10 - 30 eGFR30 - 50 eGFR50+ eGFR500 mg PO daily to BID500 mg PO BID to TID500 mg PO TID to QID500 - 1000 mg PO QID

Allergy/rash (immediate or delayed).
Can cause positive Coombs, cytopenias, eosinophilia, and hepatic and renal laboratory abnormalities.
Amoxicillin and ampicillin share a common structural side chain with cephalexin, therefore, there is an increased risk of cross allergy between these agents.
Increases metformin levels.
